ID Checklist

ID documents must meet all these requirements:

  • Be a valid (unexpired) photo ID that is government issued or issued by the school that you currently attend. School IDs from the prior school year are valid through the end of December of the current calendar year. (For example, school IDs from 2023-24 can be used through December 31, 2025.)

  • Be an original, physical document (not photocopied or electronic).

  • Bear your full, legal name exactly as it appears in your MY AP Platform.

  • Bear a recent recognizable photograph that clearly matches both your appearance on test day.

  • Be in good condition, with clearly legible English language text and a clearly visible photograph.

Acceptable ID Examples

  • Government issued driver's license or non-driver ID card

  • Official school-produced student ID card.

  • Government issued passport or U.S. Global Entry identification card

  • Government issued military or national identification card

  • AP Student ID Form: must be prepared by a Lambert official and notarized. THESE FORMS ARE ALSO AVAILABE IN THE MAIN OFFICE.

    • Use the link AP STUDENT ID FORM to download the form or pick one up in the counseling office at Lambert High School.

    • Follow the instructions, complete the form (except for school official information), and attach the appropriate PHOTO type according to the guidelines on page 2 of the form.

    • WELL BEFORE YOUR FIRST EXAM DAY, bring the completed form to the Ms. Bryant in the main office or Ms. Hosier in the counseling office. They will verify your identity and notarize the form.

    • You MUST bring this completed NOTARIZED form to each exam you take.
