Transcript Course Addition Form

  • The transcript course addition process differs by grade level for students.
  • The Transcript Course Addition Form Process for Summer 2026 will open July 16th and close August 12th.

9th, 10th, and 11th Graders

  • Can only 1.0 credit from FVA or GAVS to their transcript per semester.

12th graders

Two step process:

  1. LHS Paper Form for Transcript Course Addition
  2. LHS Online Transcript Course Addition Form for Summer 2026

12th grade students only

  • Please note BOTH steps must be completed in order for your online grade to be added to your Lambert transcript.
  • If you took classes outside of FVA or GAVS (only 12th grade students), you need to create an account on Parchment.com and request the official transcript from that school to be sent to Lambert High School.
