Transcript Course Addition Form

The transcript course addition process differs by grade level for students.

 

Deadline for all Fall 2025 Transcript Course Addition Forms: January 9th, 2026

9th and 10th graders

Can only 1.0 credit from FVA or GAVS to their transcript per semester.

Complete the FCS Online Course Student-Parent Pay Credit Acceptance Form
(transcript course addition form) in Infinite Campus Parent Portal under
Student Information-->General-->Consent-->Self Pay.

11th and 12th graders

Two step process:

1.) LHS Paper Form for Transcript Course Addition

2.) LHS Online Transcript Course Addition Form Fall 2025

Please note BOTH steps must be completed in order for your online grade to be
added to your Lambert transcript.

If you took classes outside of FVA or GAVS (only 11th and 12th grade students),

you need to create an account on Parchment.com and request the official

transcript from that school to come to Lambert High School.
